- Equipment has a potential to store energy or reaccumulate energy after shut down which could endanger employees;
- Equipment has more than one hazardous energy type;
- Equipment has the same type of energy provided from more than one source;
- The equipment must remain energized during servicing or maintenance;
- More than one lockout device is needed to lock out the equipment;
- The lockout device is not under the exclusive control of the authorized employee performing the servicing or maintenance;
- The servicing or maintenance creates hazards for other employees
- The employer, has had an accidents involving the unexpected activation or reenergization of equipment during servicing or maintenance that did not have an equipment specific LOTO procedure.
