  • Equipment has a potential to store energy or reaccumulate energy after shut down which could endanger employees;
  • Equipment has more than one hazardous energy type;
  • Equipment has the same type of energy provided from more than one source;
  • The equipment must remain energized during servicing or maintenance;
  • More than one lockout device is needed to lock out the equipment;
  • The lockout device is not under the exclusive control of the authorized employee performing the servicing or maintenance;
  • The servicing or maintenance creates hazards for other employees
  • The employer, has had an accidents involving the unexpected activation or reenergization of equipment during servicing or maintenance that did not have an equipment specific LOTO procedure.
