Quick Safety Audit

Take a moment and using the checklist below, give your organization a quick safety audit. Please feel free to Contact us for help in any areas in which you need assistance.

1- Do you have employees in California? If yes, Cal-OSHA Title 8 GISO 3203 requirements are as follows:
  • A- Do you have a written Injury & Illness Prevention Program (IIPP)?
  • B- Have all your employees and supervisors been trained on the IIPP?
  • C- Do you provide new hire safety orientation?
  • D- Do you maintain your safety training records?
  • E- Have you done initial and periodic safety inspections?
  • F- If you have a Safety Committee do they meet quarterly, record minutes and review inspection reports?

2- Do you have chemicals at your site? If yes:
  • A- Is there a written hazard communication program?
  • B- Is there a hazard communication training program?
  • C- Is there an SDS for every chemical at the site?
  • D- Do you have an inventory of all chemicals?
  • E- Are chemical containers properly labeled with the 2 types of information required?
  • F- Have you determined if you have chemicals that are on the Prop 65 list? (NOTE: Calif. Only)

3- Do you use fork lifts? If yes, the requirements are that:
  • A- Is there a written forklift safety program?
  • B- Are forklift operators trained initially and evaluated every 3 years?
  • C- Forklifts inspected each shift?

4- Do you have an OSHA 200 log for the past five years and an OSHA 300 log for the current year?

5- Do you have all the appropriate safety posters posted at your facility?

6- Do you transport hazardous materials?
  • A- If yes have you completed DOT HM-181 training and refresher training every 3 years?

7- Do you accept or ship hazardous materials/waste? If yes:
  • A- DOT requires employees to receive HM-126F training initially and every 3 years thereafter.

8- Do you use respiratory protective equipment? If yes:
  • A- Is there a written respiratory protection program?
  • B- Are users trained initially and annually thereafter?
  • C- Are employees fit-tested initially and annually thereafter?
  • D- Have you designated a Radiation Safety Officer?
  • E- Are all users medically cleared?

9- Do you use personal protective equipment? If yes:
  • A- Is there a written personal protective equipment program?
  • B- Are users trained?
  • C- Have you performed a hazard evaluation?

10- Is there high noise area or operation in the facility? If yes:
  • A- Has noise monitoring been done?
  • B- If noise levels are at or above 85 decibels do you have a written hearing conservation program?
  • C- Have you done annual Hearing conservation training?
  • D- Have employees received audiometric testing?

11- Can employees be exposed to blood or other bodily fluids? If yes:
  • A- Have you performed an exposure determination?
  • B- Do you have a written exposure control plan?
  • C- Have you done blood borne pathogen training?
  • D- Have your offered hepatitis-B vaccinations to your employees?

12- Do you repair or clear jams, clean or work on electrical or mechanical equipment? If yes:
  • A- Do you have a written Lock Out and Tag Out (LOTO) program?
  • B- Have you done LOTO training?
  • C- Do you have tool specific LOTO procedures in place?

13- Do you have confined spaces at your site? If yes:
  • A- Is there a written confined space entry program?
  • B- Are employees entering confined spaces trained?
  • C- Can you retrieve downed employees from a confined space?
  • D- Have you evaluated the rescue team?

14- Do you have 200 c.f. of gases or 55 gallon or 500 pounds of chemicals on site?
  • A- Have you submitted a Hazardous Materials Business Plan (HMBP) or HMMP to the Fire Department as required by your local municipal code?
  • B- Has the HMMP been updated in the last year?

15- Do you have a written evacuation program?
  • A- Is there an evacuation training program?
  • B- Are evacuations maps posted?
  • C- Do you have a headcount procedure?
 
16- Do you have an Emergency Response Team (ERT)? If yes:
  • A- Have they received training initially and annually thereafter?
  • B- Have they drilled once a quarter?

17- Do you have fire extinguishers? If yes:
  • A- Have they been recharged within the last year?
  • B- Have they been visually inspected monthly?
  • C- Have the employees expected to use them, been trained initially and annually thereafter?

18- Do you have fire hoses? If yes:
  • A- Have fire hose been hydro tested within 5 years?
  • B- Have the employees expected to use the equipment been trained?

19- Do you have fire sprinklers? If yes:
  • A- Has the sprinkler system been activated annually?
  • B- Has the riser system been inspected every 5 years?

20- Do you have asbestos at your site? If yes:
  • A- Have employees, contractors and vendors entering the building been informed?
  • B- Do you have an asbestos management program?
  • C- Have maintenance personnel received O&M training?

21- Do you have chemicals that are considered acutely hazardous by OSHA?
  • A- Is the amount per process greater than the Threshold Quantity?
  • B- Have you generated a Process Safety Management plan if greater than TQ?

22- Do you have chemicals on the SARA or CERCLA list? If yes:
  • A- Is the amount greater than the TPQ?
  • B- Do you know the RQ for each CERCLA/SARA chemical?

23- Do you have chemicals that are on the Cal-OSHA carcinogen user list? If yes:
  • A- Have you registered it with Cal-OSHA?

24- Do you have underground storage tanks? If yes:
  • A- Do you have a UST maintenance program?

25- Do you work in or around excavations? If yes:
  • A- Do you contact USA-ALERT prior to beginning excavation operations?
  • B- Do you have designated trained competent persons?

26- Do you have an R&D lab? If yes:
  • A- Do you have a written chemical hygiene plan?
  • B- Have you designated a Chemical Hygiene Officer?

27- Are employees exposed to chemicals? If yes:
  • A- Have you done industrial hygiene monitoring to determine if exposure is at or above the allowable limits (PEL)?

28- Do you have ventilation system or fume hoods? If yes:
  • A- Have you checked the capture velocity annually?
  • B- Are ventilation measurement records kept for the last 5 years?

29- Do you generate hazardous waste? If yes:
  • A- Do you Inspect waste area weekly and document inspections as required by EPA?
  • B- Have you completed hazardous waste handling training initially and, annual refresher training?
  • C- Do you have the last 3 years of manifests?
  • D- Do you have a waste minimization program?

30- Do you treat hazardous waste on site? If yes:
  • A- Have you submitted your Notice Of Intent?

31- Do you enter Superfund, TSD or hazardous waste sites? If yes:
  • A- Do you generate site specific safety plans as required by GISO 5192?
  • B- Have employees received 40 hour HAZWOPER training?
  • C- Have employees received 8 hour HAZWOPER refreshers?

32- Do you have parking lots? If yes:
  • A- Do you have a storm water pollution prevention program?

33- Do you have class 3b or 4 lasers or systems with embedded class 3 or 4 lasers? If yes:
  • A- Have you designated a Laser Safety Officer (LSO)?
  • B- Is the LSO trained?
  • C- Are the laser users trained?
  • D- Is there a written Laser Safety Program?

34- Do you have Contractors that work at your site? If yes:
  • A- Have they been informed of your Hazcom program?
  • B- Have they been informed of your Confined Spaces?

35- Do you have radioactive materials or radiation producing equipment at your site? If yes:
  • A- Do you have a permit?
  • B- Have you designated a Radiation Safety Officer (RSO)?
  • C- Are the users trained initially and annually thereafter?

36- Do you have elevators at your site? If yes:
  • A- Do you have a permit (California)?
  • B- Is the emergency phone tested periodically?

37- Do you have air compressors at your site? If yes:
  • A- Do you have a permit (California)?
  • B- Is the permit posted at the compressor?

38- Do you have skylights? If yes:
  • A- Do employees come within 6 feet of it?
  • B- Do you have a fall protection program?
  • C- Do you have fall protection equipment?


39- Do you have a diesel emergency generator? If yes:
  • A- Have you registered the generator with BAAQMD (California)?
