Starting January 1, 2015 all employers will be responsible for reporting all work related fatalities within 8 hours of the occurrence and inpatient hospitalizations, amputations and losses of an eye within 24 hours of the incident.
These events can be reported to OSHA via a confidential phone number, calling your closest area office during normal business hours, or filling out a new online form that will be available on the OSHA Website
