Starting January 1, 2015 all employers will be responsible for reporting all work related fatalities within 8 hours of the occurrence and inpatient hospitalizations, amputations and losses of an eye within 24 hours of the incident. These events can be reported to OSHA via a confidential phone number, calling your closest area office during normal business hours, or filling out a new online form that will be available on the OSHA Website 24-hour OSHA hotline (1-800-321-OSHA or 1-800-321-6742) "OSHA will now receive crucial reports of fatalities and severe work-related injuries and illnesses that will significantly enhance the agency’s ability to target our resources to save lives and prevent further injury and illness.” – Assistant Secretary of Labor for Occupation Safety and Health, Dr. David Michaels. The rule is being put in place in order to prevent fatalities and serious work-related injuries and illnesses.
